Authorization of Emergency Consent to Medical Treatment

I (We), the undersigned, do hereby authorize representatives of A Magical Journey Thru Stages (Stages) to serve as agents for the undersigned to consent to any X-ray exam, anesthetic, medical or surgical diagnosis or treatment and hospital care which is deemed advisable by and is to be rendered under the general or specific supervision of any physician or surgeon licensed and on the medical staff of any hospital licensed by the State of New York whether such diagnosis or treatment is rendered at the office of said physician or at said hospital or some other site. This waiver applies only in the event that neither parent/guardian can be reached in the case of an emergency.  

I (We) also understand and agree that Stages and its representatives will not be held responsible for injuries which occur to self/child while attending or participating in any Stages function. This authorization shall remain valid for the duration of the Stages 2022-2023 season and Stages 2023 Summer Camps.  

For the safety of my child/myself as well as others, I have disclosed any and all relevant medical information regarding the performer on their profile above. I understand that failure to disclose any of the above information could result in my child's/my exclusion and/or dismissal from the production.

I also agree that entering my name and the date below will be accepted as my authorized signature of agreement to these policies.

Consent
PERMISSIONS AND POLICIES

I hereby give permission for my child to participate in A Magical Journey Thru Stages’ 2022-2023 theater experiences. My child and I understand the financial and time commitment that we are making. If asked to leave for attendance or discipline issues, no refund will be given.

PAYMENT: Participation Fee and completed registration forms are required to participate in each program. A Participation Fee Due Date is defined for each program.

CANCELLED SESSION: Stages reserves the right to cancel any session for any reason, in which case tuition for that session will be fully refunded.

 
WITHDRAWAL/REFUND POLICY: 

For withdrawal from Stage Center Performers and Top Stage Teens shows during our Season, 50% of amount paid is refundable on or before the Orientation Meeting Date for each show, minus the online processing fees. After that date, the fee is non-refundable except in the case of illness or emergency that prevents the student from participating. 

For withdrawal from Stagedoor Entertainer shows during our Season, any amount paid is refundable prior to the Orientation Meeting Date listed for each show, minus a $100 administrative fee and the online processing fees. After the Orientation date, the fee is non-refundable except in the case of illness or emergency that prevents the student from participating. 

For withdrawal from Summer Camps, any amount paid is refundable prior to the Registration Fee Due Date listed for each show or camp, minus the $100 nonrefundable deposit paid for each camp and the online processing fees. After the Registration Fee Due Date, the fee is non-refundable except in the case of illness or emergency that prevents the student from participating. 

For withdrawal from classes, all participation and processing fees paid are refundable prior to the first day of class, minus a $25 nonrefundable deposit and the online processing fee. After that date, the fee is nonrefundable except in the case of illness or emergency that prevents the student from participating.

LOCATION: Locations are listed with each program session description. Most Stages shows, classes, workshops, and Summer Camps take place at Stages theater, located at the Auditorium Center, 3rd Floor, 875 E. Main Street, Rochester, NY 14605. Some workshops and camps take place in other locations throughout the community.

PARKING: Parking at Stages is generally available behind the Auditorium Center building. Enter the parking lot from College Avenue. Drop-off and pickup information will be communicated closer to program start dates. 

Permissions
STAGES CODE OF CONDUCT

We strive every day to build character in our students, performers and technical assistants, both on and off the stage, and want to deliver the very  best learning experience for all of our participants. We have the following expectations of them to make that possible. Those who participate in STAGES activities shall:

  1. Show respect for others. Fighting, harassment or bullying, name calling, intimidation, bigotry of any kind, inappropriate language or any  other form of emotional or physical abuse or unwanted contact directed to any participant, director or volunteer, whether on or off STAGES premises, will not be tolerated. The conduct prohibited is the creation of a hostile environment by threats, intimidation or abuse, including cyber bullying (harassment or bullying through any form of electronic communication), which includes, but is not limited to, harassing, teasing, intimidating, threatening, or terrorizing another participant or staff member by way of any technological tool, such as sending or posting inappropriate or derogatory  e-mail messages, instant messages, text messages, digital pictures or images, or website postings. 

  2. Maintain a professional focus. Participants are expected to pay attention to and follow the directives of the directors and STAGES staff. Disruptive, distracting or problematic behavior that affects this learning environment will not be tolerated. Cell phones and/or electronic games may not be used during classes, rehearsals, or performances. Cell phones must remain off during these periods.

  3. Respect our theater. Participants will respect the theater facilities, costumes, props, sets, and all other STAGES property. They will pick up after themselves and work to keep the theater and classrooms clean.

  4. Be on time. Participants are expected to arrive at rehearsals, classes and performances on time and ready to get to work with all necessary props, costumes, notes or scripts.

  5. Not leave unless dismissed. Participants may  not leave the theater during a class, show or rehearsal unless excused by a director or STAGES staff or when the directors or staff dismiss the group.

  6. Dress appropriately. Participants are expected to dress appropriately for all rehearsals and instruction except where otherwise guided by the director or staff for a show’s costume or for specific program activity. Items with provocative, offensive, violent, or drug related pictures or slogans will not be permitted. Items advertising alcoholic beverages or tobacco products will not be permitted. Appropriate foot wear is expected at all times. No sandals, flip flops, boots, or bare feet will be allowed. Students should wear suitable shoes for movement such as jazz shoes or sneakers.

  7. Not engage in inappropriate displays of affection. Participants are expected to act as professionals on and off the stage at STAGES, and no unprofessional or suggestive conduct will be tolerated. Any sexual activity beyond age-appropriate and consensual hugging or kissing is considered inappropriate and will not be tolerated. Relationships between staff members/artistic contractors and participants is expressly forbidden and will result in termination of the staff member/artistic contractor.

  8. Not be under the influence of drugs or alcohol, or possess drugs, alcohol, tobacco and/or weapons of any kind at any time at STAGES. Any participant found to be under the influence of alcohol or illegal substances at any rehearsal, performance, or other STAGES function will be dismissed from the production for its full run.

  9. Never discriminate. Discrimination on the basis of race, color, religion, religious practice, gender, sexual orientation, sex, national origin, ethnic group, weight, disability, or on any other basis prohibited by law, is never allowed.

 

 

Any performer who believes that he or she has observed or experienced a violation of this Code of Conduct should report it immediately to any member of STAGES staff. Violation of any of these Code of Conduct rules may result in punishment up to and including suspension or expulsion from any STAGES production or class in which the participant is involved. The determination of a violation of this Code of Conduct, as well as any punishment therefor, is within the sole and exclusive discretion of STAGES staff.
